All new ACTPS employees must complete a Pre-employment Check Package prior to commencing employment. The purpose of the package is to:

Offers of employment are made on the basis that the applicant/employee satisfies all pre-employment checks relevant to the ACTPS and any other identified position specific requirements.

As part of the pre-employment process, potential employees are required to provide evidence of their date of birth, citizenship or permanent residency, change of name, banking, tax file number, emergency contact details, and in some circumstances qualifications or professional registrations. Potential employees should complete the package as soon as possible following an offer of employment and return original forms with certified copies of requested documents to either the requesting manager/supervisor or to the Recruitment Services Team at Shared Services (see Package for address details).

Due to the size of the Pre-employment Check Package, the contents of the Package have been divided into six sub-headings. Please print, read and complete the necessary documents under all six headings.


Pre-employment Check Package for ACT Public Service applicants:

 1. Employee Details and General Obligations of Public Employees Forms Mandatory   Rich Text Format RTF (715 KB)  or   Portable Document Format PDF (95 KB)

 5.  Tax File Number Declaration Form

Tax deductions from salary are made automatically. Tax will be deducted at the highest rate if you do not provide your tax file number. To ensure that you are taxed at the correct rate you should complete a Tax File Number Declaration Form. The Australian Tax Office only provides this form in hardcopy format. Potential employees are encouraged to obtain the form from the Tax Office or selected newsagents and return with other pre-employment documents.

       
 6.  Criminal History Record Checks and Self Disclosure Form - Mandatory

All potential employees are to complete a Criminal History Record Check and a Self Disclosure Form. The type of Criminal History Record Check which is to be undertaken is dependent upon the nature of the work a potential employee is to undertake and may vary between or within ACTPS agencies/departments depending on the type of position.

       
6.1 Criminal History Record Check
 

ACT Government departments/agencies use three different types of Criminal History Record Check forms. Based on the information provided below please complete the appropriate form for your category of employment and the employing department/agency.

       
 
  • AFP Full Exclusion Form - Use this form if you are applying for one of the following types of positions:

    • ACT Health employee
    • Department of Disability, Housing & Community Services employee
    • Teacher at the Canberra Institute of Technology
    • Bus Driver with ACTION
    • Special needs transport driver/attendant/officer with ACTION
    • Legal Officer with Department of Justice & Community Safety
    • Custodial Officer with Department of Justice & Community Safety
    • Fire Fighter / Ambulance Officer with Department of Justice & Community Safety
    • InTACT employee.

6.2 Self Disclosure Form
 

In addition to a Criminal History Record Check, all potential employees are required to complete a Self Disclosure Form for matters that may be relevant to the assessment of suitability for employment but are not recorded as criminal convictions.
